The Infant Mortality Rate (IMR) in the country has reached a historic low of 25, reflecting a significant decrease of 37.5 per cent from 40 in 2013, as per the Sample Registration System report for 2023 by the Registrar General of India. The report is prepared on the basis of number of infant fatalities per 1000 births, in one year.
It means, the 2023 report indicates that in every 1000 live births in one year, 25 infants could not survive. These numbers were 40 in 2013. The lower the number the better the health accessibility.

The SRS 2023 report, based on data from 2023, indicates a sharp decline of 80 per cent in IMR from 129 in 1971. Madhya Pradesh, Chhattisgarh, and Uttar Pradesh recorded the highest IMR at 37, while Manipur had the lowest at 3.
Kerala was the only large state among the 21 to report a single-digit IMR of 5, making it second after Manipur in the country.
The report indicates a nationwide decline in IMR, dropping from 44 to 28 in rural areas and from 27 to 18 in urban areas—marking decadal reductions of about 36 per cent and 33 per cent, respectively.
Additionally, the report highlights a decrease in birth and death rates in the country.
The birth rate, a crude measure of population fertility, is a key determinant of population growth and represents the number of live births per thousand population in a specific region and year.
“The Birth Rate at all India level has declined drastically over the last five decades from 36.9 in 1971 to 18.4 in 2023. The rural-urban differential has also narrowed over these years. However, the Birth Rate has continued to be higher in rural areas compared to urban areas in the last five decades,” an excerpt of the report was quoted by PTI.
Birth Rate Declines By 14 Per CentÂ
Furthermore, the report also mentions that the birth rate declined by about 14 per cent in the last decade, from 21.4 in 2013 to 18.4 in 2023, with rural areas showing a drop from 22.9 to 20.3 (approximately 11 per cent decline). In urban areas, it fell from 17.3 to 14.9 (about 14 per cent decline).
Bihar reported the highest birth rate at 25.8, whereas the Andaman and Nicobar Islands had the lowest at 10.1 in 2023.
The death rate has steadily declined over the past five decades from 14.9 in 1971 to 6.4 in 2023.
In rural areas, it decreased from 7.2 in 2022 to 6.8 in 2023. In urban areas, it reduced from 6.0 in 2022 to 5.7 in 2023.
Chandigarh reported the lowest death rate at 4, while Chhattisgarh had the highest at 8.3, according to the report.
