The ongoing Iran–US–Israel conflict has dealt a severe blow to Iran’s top leadership, with a series of high-profile killings targeting the country’s political and military command structure. While US President Donald Trump has claimed the war has effectively led to a “regime change,” Tehran has moved quickly to replace key figures and maintain continuity in its operations.At the top of the list is Ali Khamenei, Iran’s Supreme Leader since 1989, who was killed on February 28, in a strike on a high-level meeting in Tehran in the opening hours of the war. His death marked the most significant blow to the Islamic Republic’s leadership in decades.Another major loss was Ali Larijani, Iran’s national security chief and a longtime pillar of the system, who was killed on March 17, 2026, in an Israeli strike near Tehran. His death is widely seen as the second-biggest setback for the Iranian establishment after Khamenei.Overall, the Iran–US–Israel conflict has escalated into a multi-front confrontation, with continued strikes, missile attacks and widening regional fallout even as diplomatic efforts remain uncertain. Iran has denied taking part in mediation initiatives, including those proposed by Pakistan, while continuing retaliatory attacks across the region. Amid the escalation, US President Donald Trump claimed Washington was in “serious discussions” with a “more reasonable” leadership in Iran, but warned of military action if talks fail and the Strait of Hormuz is not reopened, saying the US could target key oil and power infrastructure. A statement, Iran has denied, saying, there have been “no direct talks”.   • Ali Khamenei — Supreme Leader
    Iran’s top authority since 1989, killed on February 28 in a strike on a high-level meeting in Tehran in the opening hours of the war. The attack also reportedly killed close family members. His son Mojtaba has since taken over but has not appeared in public.
  • Ali Larijani — National security chief
    A longtime power broker within the system, killed on March 17 in an Israeli strike near Tehran that also claimed family members. His death is seen as one of the biggest institutional losses after Khamenei.
  • Mohammad Pakpour — IRGC commander-in-chief
    Former ground forces head who took over the Guards in 2025; killed on February 28, the first day of the war, and later replaced by Ahmad Vahidi.
  • Alireza Tangsiri — IRGC naval chief
    A veteran commander and key face of Iran’s maritime strategy since 2018; died on March 26 from wounds sustained in an Israeli strike, with Israel linking him to Hormuz-related operations.
  • Ali Shamkhani — Senior defence adviser
    A central figure in Iran’s security establishment since the 1980s; killed in an airstrike on February 28 after previously surviving an earlier strike.
  • Esmail Khatib — Intelligence minister
    A cleric heading Iran’s intelligence apparatus since 2021; killed in a strike in Tehran on March 18, and widely seen as pivotal to internal security operations.
  • Aziz Nasirzadeh — Defence minister
    A former air force commander and Iran-Iraq war veteran; killed on February 28 in strikes targeting senior leadership.
  • Gholamreza Soleimani — Basij commander
    Head of the Basij paramilitary force known for internal security roles; killed in an airstrike on March 17.
  • Ali Mohammad Naini — IRGC spokesperson
    Killed on March 20 in what Iran described as a US-Israel strike, shortly after publicly asserting the strength of Iran’s missile programme.
  • Mohammad Shirazi — Head of military office
    Responsible for coordinating between Iran’s armed forces under the Supreme Leader’s office; killed on February 28.
